Atenção farmacêutica (AF) é essencial para a ampliação de habilidades e interações com os pacientes com intuito de atingir melhores frutos, pois se sabe que a prevalência de doenças crônicas degenerativas em idosos é de uma certa forma exagerada, e que isso desencadeia tratamentos medicamentosos prolongados ou ininterrupto. Diante disso, objetiva-se com essa revisão de literatura, evidenciar a Atenção Farmacêutica como provedora do uso racional de medicamentos ao paciente idoso; discorrer sobre o avanço e a definição da AF; abordar o envelhecimento e a saúde do idoso e analisar a AF ao paciente idoso. Trata-se de uma revisão de literatura, por meio do levantamento e consultas nos sites científicos, como Scientific Eletronic Library online (SciELO), Literatura Latino-americana do Caribe em Ciência da Saúde (LILACS). Ao longo do processo histórico surgiu, uma nova realidade, visando a necessidade de se idealizar a AF (o cuidado que um paciente deve receber, de forma responsável, para se obter o uso racional de medicamentos e fornecer qualidade de vida ao paciente), termo que Hepler e Strand (1990), na década de 90 utilizaram pela primeira vez na literatura científica, destacando a relação dialógica entre o profissional, responsável pelo uso discriminado e racional do produto seguindo seus conhecimentos e técnicas adquiridas, somando – se a isto discorreu – se nesse trabalho, o avanço da AF, bem como sua definição, e abordou - se o envelhecimento do idoso e a sua saúde, bem como deve-se aplicar a AF na situação desses pacientes, demonstrou - se como se encaminha a AF no Brasil, com atenção especial aos idosos, de modo que se tenha clara a ideia de que em âmbito nacional, esta ainda não é uma realidade cotidiana.

Pharmaceutical care (PA) is essential for the expansion of skills and interactions with patients in order to achieve better results, since it is known that the prevalence of chronic degenerative diseases in the elderly is somewhat exaggerated, and that this triggers drug treatments prolonged or uninterrupted (CARDOSO, PILOTO, 2015). In view of this, it is objectified with this literature review, evidence Pharmaceutical Care as a provider of the rational use of drugs to the elderly patient; talk about the feedrate and AF setting; to address the aging and the health of the elderly and to analyze the AF in the elderly patient. This monographic work deals with a review of the literature, through the survey and consultations in scientific sites, such as Scientific Electronic Library online (SciELO), Latin American Caribbean Literature in Health Science (LILACS), throughout the historical process (the care that a patient should receive, in a responsible way, to obtain the rational use of medicines and provide quality of life to the patient), a term that Hepler and Strand (1990). In the 1990s, they used for the first time in the scientific literature, highlighting the dialogical relationship between the professional, responsible for the discriminated and rational use of the product, following their knowledge and acquired techniques. as well as its definition, and it was approached the aging of the elderly and their health, as well as AF should be applied in the situation of these patients, demo it was pointed out how AF is being referred to in Brazil, with special attention to the elderly, so as to make clear the idea that at the national level this is still not a daily reality.
