A Fístula Bucossinusal é uma ocorrência patológica comum, caracterizada pela comunicação do seio maxilar com a cavidade bucal principalmente depois de extrações dentárias de elementos superiores posteriores, cujas raízes possuem íntima relação com o seio maxilar. Diversas técnicas cirúrgicas são utilizadas para o fechamento de fístulas bucossinusais e a sua escolha deve ser baseada no tamanho da comunicação, nas condições locais do tecido e habilidade do cirurgião dentista. O tratamento deve ser efetuado o mais precocemente possível, evitando- se a infecção do seio e instalação de uma sinusite maxilar. Este relato de caso tem por objetivo descrever uma técnica cirúrgica alternativa minimamente invasiva para o fechamento das fístulas bucossinusais utilizando a fibrina rica em plaquetas e leucócitos (L-PRF). Para isso, os autores apresentam o caso clínico de um paciente com fístula bucossinusal pós-exodontia do segundo molar superior. O tratamento cirúrgico da fístula bucossinusal com L-PRF apresentou resultado satisfatório, com total fechamento da fístula e formação óssea após 2 anos de acompanhamento. Algumas vantagens observadas foram um rápido fechamento epitelial da mucosa, proteção da profundidade do sulco e menos dor e edema no pós-operatório. Estudos clínicos randomizados avaliando diferentes técnicas são necessários para confirmar os benefícios encontrados.

Oroantral fistula is a common pathological occurrence characterized by the communication of the maxillary sinus with the buccal cavity mainly after the dental extractions of posterior superior elements whose roots have an intimate relation with the maxillary sinus. Several surgical techniques are used for the closure of oroantral fistula and their choice should be based on the size of the communication in the local tissue conditions and the skill of the dentist surgeon. Treatment should be made as early as possible, avoiding infection of the sinus and the rise of a maxillary sinusitis. This case report aims to describe a minimally invasive alternative surgical technique for the closure of oroantral fistula using platelet-rich fibrin and leukocytes (L-PRF). For this, the authors present the case of a patient with oroantral fistula pos-surgery of the second superior molar. Surgical treatment of the oroantral fistula with L-PRF presented satisfactory results, with complete closure of the fistula and bone formation after 2 years of follow-up. The advantages observed were a rapid epithelial closure of the mucosa, protection of the depth of the sulcus and less postoperative pain and swelling. They are needed randomized clinical studies evaluating different techniques to confirm the benefits found.
