RARO CASO DE LINFANGIOMA ORAL EM PALATO: IMPORTÂNCIA DO DIAGNÓSTICO E CONDUTA DO CIRURGIÃO-DENTISTA

Os linfangiomas são malformações hamartomatosas
congênitas do sistema linfático,
pouco frequente, que diferentes autores
consideram neoplásicas. Quando presentes
na cavidade oral distribuem-se, preferencialmente,
nos dois terços anteriores
da língua, seguido pelo rebordo alveolar,
lábios e mucosa jugal. O objetivo do presente
estudo é relatar um raro caso de linfangioma
oral em palato, dando enfâse na
revisão dos conceitos atuais do diagnóstico
destas lesões. Paciente, gênero masculino,
melanoderma, 40 anos, procurou
atendimento odontológico com queixa de
“ferida no céu da boca”. Ao exame intraoral
foi observado um leve aumento de volume
nodular, séssil, eritematoso, bem delimitado,
com consistência amolecida, medindo
aproximadamente 0,5 cm, localizado em
região de palato duro. Diante das características
clínicas foi levantada a hipótese de
hemangioma. O paciente foi submetido à
biópsia excisional. O estudo histopatológico
revelou a presença de numerosos e amplos
espaços vasculares de paredes delgadas,
localizados marcadamente em posição subepitelial.
A partir destes achados foi determinado
o diagnóstico de linfangioma. O paciente
encontra-se sob acompanhamento
sem sinais de recidiva da lesão. Apesar de
ser uma lesão rara, é importante enfatizar a
importância do correto diagnóstico clinico-patológico, levando em consideração que o
prognóstico depende do tamanho da lesão,
sua localização e as implicações estéticas.

Lymphangiomas are uncommon hamartomatous
malformations of the lymphatic
system, which diff erent authors consider
neoplastic. When present in the oral cavity,
they are preferentially distributed in the
anterior two-thirds of the tongue, followed
by the alveolar ridge, labial lips and mucosa.
The objective of the present study is to
report a rare case of oral lymphangioma
on the palate, emphasizing the revision of
the current concepts of the diagnosis of
these lesions. Patient, male gender, melanoderma,
40 years old, sought dental care
with complaint of “wound in the roof of the
mouth”. On intraoral examination, a slight increase in nodular volume, sessile, erythematous,
well delimited, with softened
consistency, measuring approximately 0.5
cm, located in the hard palate region was
observed. In view of the clinical characteristics,
the hypothesis of hemangioma was
raised. The patient was submitted to excisional
biopsy. The histopathological study
revealed the presence of numerous and
wide vascular spaces of thin walls, located
markedly in subepithelial position. From
these fi ndings the diagnosis of lymphangioma
was determined. The patient is under
follow-up without signs of recurrence of
the lesion. Although it is a rare lesion, it is
important to emphasize the importance of
the correct clinicopathological diagnosis,
considering that the prognosis depends on
the size of the lesion, its location and the
aesthetic implications.
