O escroto agudo é caracterizado pela presença de dor local súbita e intensa, edema e alteração da
consistência das estruturas da bolsa testicular. Acomete mais crianças e adolescentes. As principais
causas são: torção testicular, orquiepididimite e torção de apêndice testicular. Destas, as mais freqüentes
e as que apresentam condutas mais divergentes são as duas primeiras; mas são as que mais apresentam
dificuldades no diagnóstico diferencial.
O exame mais utilizado para confirmação diagnóstica da torção do testÃculo é a ultra-sonografia do
escroto, com doppler colorido.
Manobras manuais no atendimento precoce podem reposicionar o testÃculo, desfazendo a torção com
alÃvio imediato da dor, porém não substitui o tratamento cirúrgico definitivo. As demais causas devem
ter seu tratamento direcionado ao diagnóstico.
O escroto agudo deve ser considerado uma potencial emergência cirúrgica por apresentar grande risco
de infarto testicular com conseqüente perda de função que pode se desenvolver em algumas horas.

The acute scrotum is distinguished by a sudden and intense local pain, edema and by a change in
the structures consistence of the external pouch. It happens more frequently to children and adolescents.
The mais causes are: testicular twist, orchiepididymitis and testicular appendix twist. From these,
the most frequent, which present the most divirging approachs are the first two; but they are also the
ones which are the most difficult to establish a differential diagnosis. The examination most used to
confirm a testicular twist is a scrotal ultrasound with coloured Doppler. Manual manipulations in early
attending can replace the testis by undoing the twist, which brings an imediate relief of pain. But a
definitive surgical treatment is necessary. The other causes must have their treatment attached to the
diagnosis.
Acute scrotum must be considered as a potential surgical emergency because of the high risk of testicular
infarct with consequent loss of function which can happen in a few hours.
