As demandas da doença de Alzheimer sobre a estrutura familiar

A Doença de Alzheimer (DA) é considerada também um problema da família, considerando que não afeta apenas o idoso, mas toda a estrutura familiar, levando-a a modificações na rotina, na dinâmica e nas relações, a depender do estágio da doença em que se encontra o portador. Conhecer as relações familiares, as estratégias de convivência e as respostas das famílias aos diferentes estágios da doença são a proposta deste estudo. A amostra foi constituída de 21 famílias que têm idosos com Alzheimer, selecionados com base no universo de idosos vinculados ao Programa Estadual de Medicação de Alto Custo (PEMAC), em Itabuna, Bahia, com utilização de entrevista estruturada. Constatou-se que os diferentes estágios da doença determinam às famílias adequação nas estratégias de convivência, no ambiente domiciliar e nas relações estabelecidas entre os familiares. Essas alterações ocorrem em três esferas (financeira, ambiental e humana) e já acontecem desde o estágio inicial, acentuando-se a partir do segundo e terceiro estágios. Isto tem implicado em comprometimentos importantes para as famílias, que precisam adaptar o interior das residências, absorver despesas que excedem os rendimentos dos doentes (quando têm), ou incorporá-las totalmente como custos adicionais, recorrendo a outros familiares para pedir ajuda, além da saída prematura do mercado de trabalho (ou redução da carga horária) de membros da família para envolver-se com o doente de Alzheimer, situações que reconfiguram a dinâmica das famílias, deixando-as fragilizadas e vulneráveis à doença.

Alzheimer's Disease is also considered a family problem, considering that it not only affects the elderly but the whole family structure, causing it to change the routine, dynamics and relationships, depending on the stage of the disease. The purpose of this study is to know the family relationships, the strategies and responses of families to coping with different stages of the disease. The sample consisted of twenty-one families who have elderly people with Alzheimer's, selected from the population of elderly linked to the State Program for High Cost Medications (Pemac) in Salvador, Bahia. The method used was the structured interview. It was found that the different stages of the disease determine the adequacy of household coping strategies in the home environment and the relations between family members. These changes occur at three levels (financial, environmental and human), from the initial stage to widening from the second to third stages. This has involved important compromises for family members who need to adapt the houses, to take on costs that exceed the income of patients (if they ever have one), or to support them completely, depending on other relatives for help, in addition to early withdrawal from the labor market (or reduction in hours). All these situations reshape the dynamics of families, leaving the family member in charge of caring for the AD patient fragile and vulnerable to disease.
