O presente artigo se propõe a analisar a práxis do aconselhamento pastoral no contexto hospitalar em uma relação com a elaboração do testamento vital. A análise da realidade de cuidado dentro do contexto da saúde, incluindo o cuidado espiritual como um dos importantes componentes, principalmente se tratando de pacientes terminais ou em cuidados paliativos, nos mostra a importância do tema a ser trabalhado. O confronto com a realidade da morte faz com que pacientes vivam entre o medo e a insegurança quanto ao futuro, mas também quanto a forma como serão cuidados na terminalidade da vida. A legalização do testamento vital no contexto brasileiro se torna um novo instrumento de tomada de decisões, quanto aos tipos de cuidados médicos que serão recebidos pela pessoa enferma em caso de não poder mais se manifestar. A dignidade de vida, também no final da mesma, é um tema de preocupação para a maioria das pessoas. O testamento vital pode amenizar esse sentimento e garantir legalmente um cuidado digno e desejado.

This article proposes to analyze the praxis of pastoral counseling in the hospital context in a relation with the elaboration of the living will. The analysis of the reality of care within the context of health, including spiritual care, as one of the important components, especially in terminal patients or in palliative care, shows us the importance of the topic to be worked on. The confrontation with the reality of death makes patients live between fear and insecurity about the future, but also about how they will be cared for in the terminality of life. The legalization of the living will in the Brazilian context becomes a new instrument of decision-making regarding the types of medical care that will be received by the sick person in case he can no longer manifest himself. The dignity of life, also at the end of it, is a subject of concern to most people. The living will can soften this feeling and legally guarantee a dignified and desired care.
