Durante os últimos anos, cresceu o uso dos medicamentos off label, definidos como
aqueles utilizados para indicações terapêuticas não previstas no registro concedido pelos
órgãos regulatórios. Um exemplo recente é o uso do Bevacizumabe no tratamento da
degeneração macular relacionada à idade (DMRI), doença responsável pela perda da
visão associada ao envelhecimento. Objetivo: Relatar o uso de Bevacizumabe como
medicamento off label na terapêutica da DMRI. Método: Trata-se de estudo de revisão
da literatura narrativa realizado em periódicos nacionais e internacionais, com os
descritores bevacizumabe, off label, degeneração macular relacionada à idade e
farmacoeconomia. Resultados: A pesquisa apurou a descrição do uso do Bevacizumabe
para DMRI em protocolos clínicos e artigos científicos, dentre os quais destacaram-se um
estudo clínico envolvendo 1200 pacientes e um instrumento legal do governo brasileiro
recomendando o uso do Bevacizumabe para o tratamento de DMRI. Conclusões: O uso
da droga estudada na forma de injeção intravítrea (IV) para combate à DMRI, apresentase
efetivo, com boa margem de segurança, e mecanismo de ação compatível quando
comparado a outras drogas. Destaca-se também a avaliação farmacoeconômica que
trouxe resultados positivos quanto ao uso do Bevacizumabe no tratamento da DMRI.

During the last years, have increased the use of off label medicines, known as those used
for therapeutic indications not covered by the registration granted by the regulatory
agencies. A recent example is the use of Bevacizumab in the treatment of Age-related
Macular Degeneration (AMD), a disease responsible for age-related vision loss.
Objective: Describe the use of off label medicine on therapy, with emphasis to the
Bevacizumab on AMD. Method: It is the study of the literature review carried out in
national and international periodicals, performed with the Bevacizumab, off label and
Age-related Macular Degeneration (AMD) descriptors. Results: The research found the
description of Bevacizumab use for AMD in clinical protocols and scientific articles,
among which stood out the clinical study involving 1,200 patients; has also been
identified legal instrument of the Brazilian government recommending Bevacizumab for
the treatment of AMD. Conclusions: The use of the drug studied in the form of
intravitreal injection (IV) to struggle against AMD, demonstrated effectiveness, with
good safety margin, and compatible mechanism of action when compared to other drugs.
Also noteworthy is the pharmacoeconomic evaluation that brought positive results
regarding the use of Bevacizumab in the treatment of AMD.
