A fístula gastrocólica é uma comunicação entre o estômago e o cólon com etiologias variadas, incluindo patologias benignas e malignas, que acomete mais mulheres idosas. Seus sintomas são inicialmente inespecíficos, dificultando o diagnóstico e retardando o tratamento adequado, geralmente cirúrgico. Apresentamos um caso de evolução fatal, com diagnóstico tomográfico que ilustra radiologicamente a condição, apontando para sinais que devem ser reconhecidos por clínicos e radiologistas.

Gastrocolic fistula is a communication between the stomach and colon with varied etiologies, including benign and malignant pathologies, which affect more commonly elderly women. Its symptoms are initially nonspecific, delaying diagnosis and adequate treatment, usually surgical. We present a case of fatal evolution, with tomographic diagnosis that radiologically illustrates the condition, pointing to signs that should be recognized by clinicians and radiologists.
